Filing Information and Application Forms

These forms and instructions are the minimum requirements for a filing with The Nantucket Conservation Commission, 131 Pleasant Street, 2nd Floor, Nantucket, MA, 02554.

Notice of Intent 

When filing a Notice of Intent the following materials are required:

  • Two copies of the written request/application form
  • Certified abutters list no older than six months.
  • Copies of the abutter and DEP certified mailing receipts(NHESP if applicable )
  • Two full size copies of the plans
  • One reduced sized copy of the plan (8.5" X 11")
  • Reduced size PDF digital version emailed to Will DellErba and Lisa Graves.

Filing fees (must be separate checks):

  • $25.00 - Town of Nantucket (Bylaw Fee)
  • $200.00 - Town of Nantucket (Reviewer Fee)
  • $184.71 - Inquirer and Mirror (Advertising Fee)
  • $Misc - Town of Nantucket (City Share Fee of Wetland Transmission Form)

Amended Orders of Conditions

When filing a request to amend an Order of Conditions the following materials are required:

  • Two copies of the written request/application form
  • Certified abutters list no older than six months with a copy of the certified mailing receipts
  • Two full size copies of the plans
  • One reduced sized copy of the plan (8.5" X 11")
  • Copy of first page of Original Order showing registered information
  • Reduced size PDF digital version emailed to Will DellErba and Lisa Graves.

When filing a Request for Determination of Applicability the following materials are required:

  • Two full size copies of the plan
  • One reduced size copy of the plan (8.5" X 11")

  • Reduced size PDF digital version emailed to Will DellErba and Lisa Graves.

When filing a request for a Certificate of Compliance the following materials are required:

  • Two copies of the written request/application form to be filed at Nantucket Conservation Commission.
  • Two copies of the full size as-built plan that shows resource areas and buffer zones.
  • Copy of first page of Original Order showing registered information.
  • One reduced size copy of the plan (8.5" X 11")
  • Reduced size PDF digital version emailed to Will DellErba and Lisa Graves.

Minor Modification/Plan Change

When filing for a Minor Modification the following information is required:

  • Two copies of the written request/application form.
  • Two copies of the full size as-built plan that shows resource areas and buffer zones.
  • One reduced size copy of the plan (8.5" X 11").
  • Reduced size PDF digital version emailed to Will DellErba and Lisa Graves.
  • Filing Fee $25.00 - Town of Nantucket

Request for Extension of Orders of Conditions

When filing for a Extension of Orders of Conditions the following information is required:

  • A letter stating the reason as to why an Extension is being requested along with the number of years you would like to extend i.e (1 yr or 3 yrs)
  • One copy of first page of Original Order showing registered information and a copy of the Findings & Conditions page.
  • One completed copy of the Request for Extension form below
  • Reduced size PDF digital version emailed to Will DellErba and Lisa Graves.
